Welcome to Lock and Key Wellness, LLC (LKW). LKW provides confidential services for individual and family wellness coaching. This document outlines important information about our professional services and business policies, including our commitment to your privacy and confidentiality. LKW is dedicated to maintaining the confidentiality of all personal information disclosed during our wellness coaching sessions. We uphold a standard of confidentiality that respects and protects your privacy in all interactions and communications.
Any personal information you share with us will be used solely for the purpose of providing you with the best possible wellness coaching experience. Information will only be disclosed with your explicit consent or in specific circumstances where disclosure is required by law, such as in cases of imminent harm to yourself or others, or as may be otherwise legally mandated.
We encourage open and honest communication, knowing that the information shared is treated with the utmost respect for your privacy and confidentiality. Should you have any questions or concerns about our confidentiality practices, please do not hesitate to discuss them with us.
The information and all services provided such as, but not limited to, coaching, education, guidance by LKW is to be used for educational purposes only and does not constitute legal or professional advice. You are agreeing to disclose all relevant information regarding physical, emotional and/or mental conditions that could cause harm to you by participating in these coaching and educational services. LKW is not acting as a mental health counselor or a medical professional in any capacity in these services. The coaching and educational services from LKW are not offered as a substitute for professional mental health care or medical health care and are not intended to diagnose, treat, or cure any mental health or other medical conditions. Wellness coaching is, at present, an unregulated industry and LKW is not licensed by any state. For all legal purposes, all the coaching and educational services provided by LKW will be provided in the State of Florida. Wellness coaching provided by LKW is without warranty. LKW is not promising outcomes. All comments, ideas and suggestions offered by LKW are opinions that are expressed solely for the purpose of helping and educating you towards achieving goals.
WELLNESS COACHING PROCESS AND SERVICES.
Wellness coaching is an alliance between client and coach to increase understanding and bring about change. The coaching process varies depending on the personalities of the coach and client and the particular problems you are experiencing. There are many different methods LKW may use to deal with the problems that you hope to address. Wellness coaching calls for a very active effort on your part. For the coaching to be most successful, you will need to work on the things we talk about both during our sessions and independently.
Wellness Coaching can have benefits and risks. Since coaching often involves discussing unpleasant aspects of your life, you may experience uncomfortable feelings like sadness, guilt, anger, frustration, loneliness, difficulty concentrating, and helplessness. On the other hand, wellness coaching has also been shown to have many benefits including the goal to seek joy in your daily life. Wellness coaching often leads to better relationships, solutions to specific problems, and significant reductions in feelings of distress. Wellness coaching can help individuals change patterns of thinking, feeling, and behaving, so they can create a life that is more satisfying and fulfilling. Although there are no guarantees of what you will experience, I will devote my attention to ensure that we maintain a safe and respectful environment that can maximize the possibilities for you to achieve positive growth and healing. We will work together to determine the pace and form of treatment.
Our first few sessions will involve an evaluation of your needs, goals, and circumstances. By the end of the evaluation, I will be able to offer you some first impressions of what our work will include and a treatment plan to follow if you decide to continue with wellness coaching. You should evaluate this information along with your own opinions of whether you feel comfortable working with me. Wellness coaching involves a large commitment of time, money, and energy, so you should be very careful about the coach you select. If you have any questions about my procedures, we should discuss them whenever they arise. If your doubts persist, I will be happy to help you set up a meeting with another wellness coach for a second opinion or referral.
COACHING SESSIONS / MISSED APPOINTMENTS.
If Wellness coaching is begun, sessions will be scheduled at a frequency that is mutually agreed upon. We may also decide to change the length and frequency of sessions at any time during the course of your coaching. Once an appointment is scheduled, you will be expected to pay for the full fee unless you provide 24-hours (1–day) advance notice of cancellation. I understand illnesses, accidents, and other beyond-our-control situations occur, and with these events, there will be no charge as long as you contact the office of such an issue. If you appear in a session under the influence of alcohol or other substances, then I will cancel the session, find you a safe ride home if needed, and require you to pay for the session.
Confidentiality. In general, the privacy of all communications between a client and wellness coach are protected by law; we can only release information about our work to others with your written consent. There are a few exceptions to the confidentiality laws and standards, as follows :
- If your wellness coach believes that you or someone else is in clear and eminent danger of harm, your coach is legally obligated to inform proper authorities and others in order to help prevent the harm from

It is also possible that at some point in the future you will be required by an outside agency to sign a release allowing the agency to review your treatment records. This may occur, for example, if you apply for life insurance, licensure or certification in some professions, or if you require a security clearance.

Professional Consultations. To provide you with the best possible service, your wellness coach may choose to consult with other professional staff or colleagues that are subject to the same confidentiality constraints that your coach is.
Risks of Wellness coaching. Research indicates that most people who engage in wellness coaching benefit from the experience; even so, it is possible for things to get worse before they get better. For example, it is not uncommon for clients to make changes in their interpersonal relationships as a result of wellness coaching, and such changes can sometimes be difficult. In addition, some clients who work on especially troubling issues may find it difficult to concentrate. You and your coach will work together to determine the pace and form of treatment to minimize the risks while maximizing the benefits of wellness coaching.
Missed Appointments. Please give us as much notice as possible if you have to miss an appointment. Should you miss an appointment without calling 24 hours in advance the regular hourly fee will still be charged.
Billing and payments. As wellness coaching does not diagnose, treat, or cure any medical or mental health condition, nor does it replace treatment for any such condition these services are not covered by insurance. LKW has provided you with the charges, in advance, for the services you have requested. You have been given a choice of different services, along with their costs. You have selected the services and are willing to accept full financial responsibility for payment. Payment is expected prior to each session and is nonrefundable.
Technology and Social Networking. LKW advises clients that use of e-mail or text messaging has a number of risks and LKW cannot guarantee confidentiality of this type of communication. Therefore, the client should not use email or text for communication regarding sensitive medical or mental health information. LKW provides a professional website with resources about wellness but LKW does not accept “friend requests” from current or former clients on their social networking sites since these sites can compromise client’s confidentiality and privacy. For the same reason, LKW requests that clients do not communicate with LKW via any interactive or social networking sites.
Hours of Operation. You may contact your counselor between 9 am and 5 pm Monday through Friday excluding holidays by calling (656)227-9547. If it is an emergency, you should go to the local emergency room.
Wavier. You release, waive, acquit, and forever discharge LKW, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages you may claim to have or that you may have arising out of acts or omissions by you or by LKW as a result of the educational information and advice given resulting from the coaching relationship contemplated by this agreement. This agreement shall bind my heirs, executors, personal representatives, successors, assigns, and agents.
You further declare and represent that no promise, inducement, or agreement not expressed in this agreement has been made for you to sign this agreement.
You have the legal ability to give your informed consent, and hereby give such consent to LKW to educate and assist you towards achieving your goals.
You, the client, understand and agree that by filling out your contact information below and submitting this form, that you have reviewed, understand, and agree to each of the statements and the terms and conditions stated above. You are requesting coaching and educational services from LKW.
I have read the above information and concur with the terms and conditions throughout my involvement with Lock and Key Wellness, LLC. I have also received a copy of the Privacy Practices if requested and have read and understand the Privacy Practices regarding my health information.
